Yizkor Memorial Book Name Form 5785

Every year, NVHC creates a Memorial Book to memorialize those we recognize during Yizkor on Yom Kippur. This year, our Memorial Book will be digital, allowing us the space to include photos, stories, and memories of our loved ones.

Please list the names of your loved ones you would like printed in the book below. You can also enter a short story or favorite memory of them along with a photograph if you would like (150 words). We may lightly edit your submission for grammar or clarity.

If you submitted information for last year's Memorial Book and would like us to include the same information this year, please check the box at the top of the form.

Please list the name of your loved one along with the story. Please limit your writing to 150 words.
Please make sure the file name is the name of the person in the photo, so that we can pair the photo with the correct person.
Please only submit one photo per entry.
The suggested donation is $36 for the first name you would like to list, and $18 for each additional name. This helps to cover the cost of creating the books and providing a spiritually nourishing and meaningful Yizkor service.
