Every year, NVHC creates a Memorial Book to memorialize those we recognize during Yizkor on Yom Kippur. This year, our Memorial Book will be digital, allowing us the space to include photos, stories, and memories of our loved ones. This year, we will also present the book as a slideshow for everyone to view.

Please list the names of your loved ones you would like printed in the book below. You can also enter a short story or favorite memory of them along with a photograph if you would like (150 words). We may lightly edit your submission for grammar or clarity.

If you submitted information for last year's Memorial Book and would like us to include the same information this year, please check the box at the top of the form.

So that we have ample time to prepare the book, the deadline is September 21.
